荷兰驻华大使馆通知：自2006年5月15日起，对荷兰驻华大使馆及荷兰驻上海和广州总领馆认证的文书，荷兰使领馆将视需要附上两页调查表返回给申办人。调查表第一页(问题1-13)由荷兰使领馆填写，第二页(问题14-26)须由当事人填写(英文或荷兰文)并签名，与文书一起使用，否则文书有可能不被荷兰国内相关部门接受。

　　Important information for legalizations:

　　As of Monday15 May 2006, each document, legalised by the Netherlands Embassy in Beijing or the Consulates General in Shanghai and Guangzhou will be returned to its owner after legalisation, with an questionnaire of 2 pages.

　　Page l (questions l-13) will be completed by the Embassy/Consulate General.

　　Page 2 (questions14-26) must be fully completed and signed by the owner of the document, before use of the document in any administrative procedure in the Netherlands.

　　The use of foreign documents, legalized on or after May 15may 2006, in any procedure of Netherlands public authorities, without a fully completed and signed questionnaire, may influence the outcome of that procedure.

　　The form has to be completed in English (or in Dutch).
